Patients & Caregivers
Choosing hospice is often a difficult decision. We understand that difficulty, and are here to answer to questions and dispel myths that exist regarding hospice care.
Hospice is a plan of palliative comfort care for a loved one or patient who is declining and heading towards life transition. Our program is designed to give the best quality of life on a daily basis and focus on excellent physical, spiritual, and emotional needs and not just on a disease.
Hospice does not hurry death or prevent death. Our team of care professionals reaffirms life, focuses on dignity, and brings comfort and quality to the remaining months, weeks, and days.
We are here 24 hours a day 7 days a week 365 days of the year.
If two or more of the below possible indicators are present, hospice should be considered, and discussed with the patient, the patient’s family, and healthcare professional. Anyone can make a hospice referral. We will perform an evaluation to help determine if the patient may meet the criteria for hospice eligibility.
