Our hospice team is dedicated to providing compassionate care and extraordinary service to the patients and families we serve. Our patient-centered approach treats their physical, emotional, social and spiritual needs, based on the specific needs of the patient and family. We surround the entire family with an interdisciplinary team that specializes in end-of-life care. We are here 24 hours a day 7 days a week.
Meet Your Hospice Team
Medical Director / Hospice Physician
Provides oversight of the patient plan of care and oversees the pain and symptom management of the patient. Coordinates with the attending physician when needed.
Hospice Volunteer
Provides companionship, helps with household chores, as needed.
Social Worker
Helps bridge emotional and social gaps, and identifies community resources for the family. May also assist with end-of-life planning.
Registered Nurse / Case Manager
Provides hands-on physical care to ease symptoms and provide comfort for the patient. Coordinates care with the entire hospice care team.
CNA – Certified Nursing Assistant / Hospice Aide
Assists and provides personal care such as bathing and grooming.
Chaplain / Spiritual Counselor / Bereavement Counselor
Helps with spiritual needs as requested by the patient and family. Also offers bereavement support.
